If you are a Social Worker and are interested in being part of an exciting healthcare reform initiative, consider submitting an application to become an Emergency Department Social Worker at Niagara Falls Memorial Medical Center. The position is responsible for working with high utilizers of ED services (both mental health and medical) and connecting patients to community-based care to avoid unnecessary hospitalizations and repeat ED visits.
Duties include: Completion of patient assessments, care plans, counseling and interventions to meet the physical health, behavioral health and social determinant of health needs of ED patients and their families.
Regular work hours are: 10 a.m. to 6 p.m., Monday – Friday.
Qualifications include:
- Master’s degree in Social Work, MSW or LMSW
- Relevant experience, preferably in a healthcare environment.
- Persons with a Masters Degree in a Mental Health or an Addiction Treatment field are also eligible to apply.
